Independent Co-Founder, Rea Medical Support Group (Privately Contracted Project)

Rea Medical Support Group is a privately contracted support initiative to provide comprehensive assistance to individuals with higher medical needs, ensuring they receive personalized care and streamlined support for all aspects of their health and independent living. Our work focuses on simplifying complex healthcare processes while maintaining detailed records and fostering independence for clients. Key responsibilities include: 

• Master Documentation Management: Creating and maintaining detailed master documents that include medical histories, medication lists, updated diagnoses, and current care plans, ensuring all information is accurate and easily accessible.

• Appointment Coordination and Documentation: Scheduling medical appointments, coordinating follow-ups, and documenting outcomes to ensure continuity of care. I also track notes from appointments and communicate updates with healthcare providers.

• Community and County Services Compliance: Preparing and submitting documentation to maintain eligibility for county and community services, ensuring clients remain connected to vital programs and resources.

• Medication and Health Monitoring: Tracking medication schedules, recording changes, and maintaining detailed logs of medical needs, providing clients and families with peace of mind.

• Transportation and Travel Management: Organizing transportation for local and long-distance medical appointments, including accommodations for specialized travel needs and scheduling.

• Independent Living Skills (ILS) Support: Offering tailored support to foster independence, focusing on skills that enhance the quality of life and align with individual care plans.

• Comprehensive Annual Recordkeeping: Managing all documentation for medical procedures, appointments, and services throughout the year, ensuring clients are prepared for any required audits or reviews.

• End-of-Life and Emergency Planning Support: I provided support in developing and organizing master folders, binders, and digital files containing essential life documents. This included creating structured systems that allowed for easy access during emergencies and end-of-life situations, as well as preparing and maintaining accurate digital records with typed summaries and verified copies of important documents. I also assisted families and individuals in planning next steps, such as funeral arrangements and documenting end-of-life wishes, while offering referrals and guidance to finalize paperwork with legal professionals, including attorneys and notaries.
